Job summary

Placements24 is seeking a proactive and detail-oriented Support Coordinator to join a remote team serving clients in the Klerksdorp area. You will be the primary contact for clients, families and service providers, coordinating care to help people live independently.

Responsibilities include developing and monitoring individualized support plans, arranging access to personal care, therapy and community services, and reviewing plans to ensure they meet goals.

Qualifications

• Experience in a support coordination, case management, or similar role within community services.
• Solid understanding of disability services, aged care, or other relevant support frameworks in South Africa.
• Excellent organizational skills and the ability to manage multiple client cases concurrently.
• Strong communication and interpersonal skills, adept at virtual interactions.

Responsibilities

• Develop, implement, and monitor individualized support plans for clients.
• Coordinate and arrange access to a range of support services, including personal care, therapy, and community access.
• Liaise regularly with clients, families, support workers, and external service providers via phone and digital platforms.
• Conduct regular reviews of support plans to ensure they remain responsive to client needs and goals.
• Maintain accurate and up-to-date client records and service documentation.
• Respond to client inquiries and concerns in a timely and professional manner.
