Placements24 is seeking an experienced Remote Case Manager to support elderly clients and their families from a home office. You will assess needs, create personalised care plans, and coordinate medical, social, and domestic services.

This role requires strong advocacy, organisation, and independence, with virtual collaboration across healthcare professionals and community resources. The position supports clients in the Western Cape and related communities through remote service delivery,

Qualifications

• Bachelor's degree in Social Work, Gerontology, Nursing or related field.
• Minimum 5 years of experience in case management, social work, or elder care.
• Experience in care planning, needs assessment, and service coordination for the elderly.
• Strong understanding of aging and geriatric conditions.
• Excellent communication, active listening, and empathy skills.

Responsibilities

• Conduct comprehensive remote assessments of clients' needs.
• Develop and review personalised care plans with clients and providers.
• Coordinate healthcare, medication, domestic assistance, and social activities.
• Serve as main liaison with clients and families, providing updates and support.
• Liaise with healthcare professionals, social workers, and external agencies.
• Monitor well-being and adjust care plans as needed.
• Maintain confidential records using secure online case management systems.
• Ensure adherence to regulations and organisational policies.
• Identify risks and implement mitigation strategies for safety.
• Provide information and referrals about available support services.
