Job summary

Placements24 is seeking a highly motivated Virtual Social Support Navigator to join our innovative team. This fully remote role focuses on guiding individuals through social service systems, connecting them with resources, and assisting with applications from home.

The ideal candidate has a background in social work or related field, strong virtual communication skills, and experience in case management to support clients across South Africa.

Qualifications

• Background in social work, counseling, community services or related field.
• Experience in case management or client navigation.
• Excellent virtual communication via video calls and messaging.
• Ability to work independently in a remote setting with strong organization.

Responsibilities

• Provide remote guidance and support to clients seeking social services and community resources.
• Assist clients in identifying needs and navigating online platforms and applications.
• Conduct virtual needs assessments via video calls, phone, or secure messaging.
• Connect clients with appropriate agencies, support groups, and healthcare providers.
• Follow up with clients to ensure they receive necessary support and address barriers.
• Maintain accurate and confidential client records using secure digital systems.
• Educate clients on available resources and entitlements relevant to their circumstances.
• Collaborate with internal support teams and external service providers to ensure seamless client journeys.
• Develop and maintain an up-to-date knowledge base of social services and community programs.
• Offer empathetic and encouraging communication to empower clients.
