Qualifications

• Qualification in Social Work, Community Health, Psychology or related field.
• At least 3 years of experience in community support or care coordination.
• Proficient with video conferencing and case management software.
• Excellent written and verbal communication for remote interaction.

Responsibilities

• Coordinate and manage remote care plans for clients.
• Conduct virtual assessments and follow-ups.
• Facilitate communication among clients, families, and providers.
• Maintain accurate client records via case management software.
• Provide information and referrals to community resources.
• Troubleshoot issues in remote care delivery.
• Develop and implement remote support protocols.
