Job summary

Placements24 is seeking an experienced Senior Care Navigator to join a remote-first team in South Africa. You will guide clients through social care systems, assess needs, and coordinate with providers to ensure access to resources.

Requires a Master’s in Social Work or related field, at least 6 years of case management experience, and strong virtual communication skills. You will mentor junior navigators and maintain accurate EHR records.

Qualifications

• Master's degree in Social Work, Public Health, or related field.
• Minimum 6 years of experience in case management or care coordination.
• Strong knowledge of social care services and community resources.
• Proven ability to manage complex cases and advocate for clients.

Responsibilities

• Provide in-depth guidance and support to clients navigating social care services.
• Conduct comprehensive needs assessments and develop personalized care navigation plans.
• Coordinate with service providers to facilitate access to resources.
• Advocate for clients' needs and promote equitable access to services.
• Maintain detailed client records using EHR systems.
• Monitor progress and adjust care plans as needed.
• Respond to client inquiries via virtual channels.
• Mentor junior care navigators within the remote team.
