Placements24 is seeking an experienced Remote Case Manager to join our social services team. The role supports clients nationwide from a home-based setting, conducting assessments and developing tailored action plans with access to community resources.

You will coordinate referrals, provide counseling and advocacy, and maintain accurate client records. The ideal candidate has 4+ years in case management, professional registration, and strong digital communication and organizational skills.

Qualifications

• Bachelor's degree in Social Work or related field required.
• Professional registration with the relevant South African council is required.
• Minimum 4 years' experience in case management or social work.
• Proven ability to conduct client assessments remotely and develop service plans.

Responsibilities

• Conduct remote client assessments via phone, video conferencing, and digital platforms.
• Develop, implement, and monitor personalized case management plans.
• Coordinate services and referrals to community and government agencies.
• Provide ongoing support, counseling, and advocacy for clients.
• Maintain confidential and up-to-date client records in the system.
• Communicate effectively with clients, service providers, and internal stakeholders.
