Placements24 is seeking a highly organized and dedicated Remote Community Case Manager to join their fully remote team, serving the Centurion area. You will coordinate a caseload, develop care plans, and connect clients with healthcare, housing, employment, and social services.

This role emphasizes remote communication, data protection, and client advocacy. The ideal candidate has a Bachelor's degree in a related field and at least 3 years of case management experience.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or related field.
• Minimum 3 years of experience in case management or similar client support role.
• Proficiency with case management software and virtual communication tools.
• Strong understanding of social service systems and community resources.

Responsibilities

• Conduct remote client assessments via phone and video conferencing to identify needs and strengths.
• Develop, implement, and monitor individualized case management plans for diverse clients.
• Coordinate access to community resources including healthcare, housing, employment, and social services.
• Maintain accurate client records in compliance with agency policies and data protection regulations.
• Regularly communicate with clients, families, and service providers to ensure continuity of care.
• Advocate on behalf of clients to secure appropriate services and support.
