Placements24 is seeking a dedicated Remote Case Manager to deliver comprehensive social services to individuals and families from a remote home office. You will assess needs, tailor action plans, and connect clients to housing, healthcare, and educational resources across the Western Cape and beyond.

The role requires autonomy, excellent communication, and a strong commitment to client wellbeing, with collaborative work across a virtual team.

Qualifications

• Bachelor's degree in Social Work, Psychology, or a related field.
• Minimum of 4 years of experience in case management or social services.
• Proficiency with virtual communication tools (e.g., Zoom, Teams) and case management software.
• Strong understanding of social service systems and resource availability.
• Excellent assessment, problem-solving, and interpersonal skills.

Responsibilities

• Conduct remote client assessments to identify social, economic, and health needs.
• Develop, implement, and monitor individualized case plans with clients.
• Connect clients with housing, employment, healthcare, and educational resources.
• Provide ongoing support, advocacy, and counseling via virtual channels.
• Maintain confidential client records per organizational policies.
• Collaborate with team members and external service providers via digital platforms.
