Job summary

Placements24 is seeking a proactive Case Manager to join its social services team. The role is initially focused on clients in the Rustenburg area with full remote flexibility, allowing autonomy while making a meaningful impact.

You will manage a caseload, assess needs, develop care plans, coordinate access to services, and advocate for clients. Strong organizational and communication skills are essential to maintain records and collaborate with partners.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related field.
• Experience as a Case Manager or in similar social services role.
• Familiarity with local social service networks in the North West province.
• Strong organizational, time management, and documentation skills.
• Excellent communication and interpersonal skills.
• Proficiency with digital communication tools and case management software.

Responsibilities

• Assess client needs and develop comprehensive, individualized case management plans.
• Coordinate services with healthcare providers, government agencies, and community organizations.
• Monitor client progress and adjust service plans as necessary to ensure effectiveness.
• Provide counseling and support to clients facing social, emotional, or economic challenges.
• Maintain detailed and accurate client records, ensuring confidentiality and compliance.
• Advocate on behalf of clients to ensure they receive appropriate services and support.
