Job summary

Placements24 seeks a highly organized Remote Case Manager to support individuals and families within the social services sector. Working remotely, you will assess needs, connect clients with resources, advocate for their rights, and monitor progress toward stability and self-sufficiency.

The role requires strong digital communication skills, independent work habits, and the ability to manage a caseload across diverse regions, including areas historically linked to Mahikeng.

Qualifications

• A Bachelor's degree in Social Work, Sociology, Psychology, or related field.
• Proven experience in case management or similar client-facing role.
• Strong understanding of social service systems, resources, and eligibility criteria.
• Excellent organizational skills and ability to manage multiple cases.

Responsibilities

• Assess client needs and develop individualized case management plans to achieve goals.
• Connect clients with social services, healthcare providers, educational opportunities, and employment resources.
• Monitor progress and adjust care plans as needed.
• Maintain accurate, confidential electronic case files.
• Collaborate with remote team, supervisors, and external agencies to ensure comprehensive client support.
• Advocate for clients' rights and access to services within community and governmental systems.
