Job summary

Highmark Health is seeking a Wellness Coach to oversee a panel of members, assessing health management needs and designing personalized care plans to improve outcomes. You will work with a multidisciplinary team, using data and clinical judgment to guide interventions and support behavior change through telephonic, digital, or virtual contact.

You will monitor quality outcomes, coordinate care across ages and regions, document activities to compliance standards, and contribute to program

Qualifications

• Master's degree or equivalent in a health-related field with experience directed to wellness.
• Experience in a wellness model, disease management, or health program delivery.
• Ability to document activities in compliance with HIPAA and policies.

Responsibilities

• Lead and coordinate wellness interventions across tobacco cessation, weight management, sleep, stress, nutrition, and activity.
• Develop personalized care plans and coordinate with multidisciplinary resources.
• Monitor outcomes, ensure compliance, and contribute to program improvements.
• Engage members by phone, digitally, or virtually to promote behavior change.
