Highmark Health seeks a Wellness Coach to lead lifestyle interventions for a defined member panel, focusing on behavior change and personalized care planning. You will engage members via telephone, digital tools, and virtual visits, coordinating with a multidisciplinary team to improve health outcomes.

The role emphasizes evidence-based interventions across nutrition, activity, sleep, and stress, with emphasis on documentation, compliance, and quality metrics in a high-performing environment.

Formación

• Master’s degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or Clinical related field or equivalent experience.
• Minimum 3 years as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment.

Responsabilidades

• Lead lifestyle coaching across tobacco cessation, weight management, sleep, stress, nutrition and physical activity.
• Assess health needs of panel members and develop personalized care plans.
• Coordinate referrals to internal and external resources to support goals.
• Document activities and ensure compliance with policies and regulations.
• Work with multidisciplinary teams to improve quality outcomes and cost savings.
