Highmark Health seeks a wellness coach to own a member panel, assess health needs, and apply data-driven clinical judgement with a multidisciplinary team. You will develop personalized care plans, encourage behavior change, and coordinate care across services to achieve improved outcomes.

The role requires experience in wellness coaching, ability to engage with members telephonically or digitally, and documenting activities in compliance with standards.

Qualifications

• Master’s degree or equivalent related education as determined by the company.
• 3 years as a Registered Dietician, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment.

Responsabilités

• Maintain knowledge of evidence-based interventions to deliver lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Oversee a specified panel of members; assess health management needs and triage to resources.
• Create care plans with long- and short-term goals and regular contact for progress reassessment.
• Demonstrate clinical outcomes and cost savings from wellness interventions.
• Ensure activities comply with regulatory and accreditation standards.
• Participate in workgroups to improve efficiency and patient-centric wellness services.
• Other duties as assigned.
