Job summary

Highmark Health seeks a Wellness/Lifestyle Coach to lead a defined panel of members with varying health statuses, assessing needs and creating personalized care plans. You will engage members telephonically, digitally, or virtually, coordinate with a multidisciplinary team, and monitor outcomes to improve health metrics.

You will use data analytics and clinical judgement to select interventions, document activities, and contribute to program development and pilots.

Qualifications

• Master's degree or relevant experience as described by the company.
• 3 years as a Registered Dietician or Exercise Physiologist or Wellness/Lifestyle Coach in a wellness-focused environment.

Responsibilities

• Maintain knowledge of evidence-based interventions across lifestyle areas.
• Oversee a panel of members, assess needs, and refer to appropriate resources.
• Create care plans with goals and regular contact for progress.
• Demonstrate clinical outcomes and cost savings from wellness interventions.
• Ensure documentation complies with policy and accreditation standards.
• Participate in workgroups to improve wellness coaching programs.
• Perform other duties as assigned.

Education

Master's Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or Clinical related field OR relevant experience
