Highmark Health is seeking a wellness lifestyle coach to oversee a panel of members with varying health needs. You will assess health management needs, tailor interventions, and coordinate with a multidisciplinary team to refer members to resources and develop personalized care plans.

You will engage members by phone, video, or online, monitor outcomes, and document activities to meet regulatory and quality standards.

Qualifications

• Master's degree in a health field or relevant experience as determined by the company.
• Minimum 3 years as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment.
• Preferred certifications include tobacco cessation, RD, CDE, exercise physiology, lactation counselor, NDP lifestyle coach, CHWC.

Responsabilités

• Deliver high-quality lifestyle coaching on tobacco cessation, weight management, sleep hygiene, stress, nutrition, physical activity, and maternal health.
• Develop personalized care plans and coordinate care across resources.
• Engage with members via phone, video, or virtual visits to improve outcomes.
• Document activities per regulatory and accreditation standards.
• Contribute to workgroups to improve wellness services and pilot new programs.
