Job summary

Highmark Health is seeking a dedicated Wellness Coach to lead a panel of members with varied health needs. You will assess health management requirements and use data and clinical judgment to tailor interventions.

You will coordinate with a multi-disciplinary team and refer members to appropriate resources while maintaining proactive outreach to improve health outcomes. You will document activities in compliance with regulatory and accreditation standards and contribute to ongoing program

Qualifications

• Master's degree or equivalent combination as determined by the company.
• Minimum 3 years as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment.
• Experience with health plans/disease management or clinical program delivery is preferred.

Responsibilities

• Maintain knowledge of evidence-based interventions to deliver lifestyle coaching across tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Create care plans for assigned members, address barriers, identify resources, and plan regular contact for reassessment.
• Engage members via telephonic, digital, or virtual means to improve health outcomes.
• Monitor and report on clinical, financial and functional outcomes and ensure documentation complies with standards.
• Participate in workgroups to improve efficiency and member-centric wellness services and pilot new programs.
• Other duties as assigned.
