Job summary

Highmark Inc. is seeking a Wellness Coach with a focus on nutrition, health promotion, and preventive care. You will manage a panel of members, assess their health needs, and design tailored care plans with ongoing follow-up to improve outcomes.

You will engage members by phone, digitally, or virtually, coordinating with a multidisciplinary team while ensuring documentation aligns with regulatory and accreditation standards. Prior experience in wellness settings is required.

Qualifications

• Master's degree or equivalent in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or Clinical related field or equivalent experience.
• 3 years as a Registered Dietitian or Exercise Physiologist or Wellness/Lifestyle Coach in a wellness-focused environment.
• Preferred certifications include Certified Tobacco Cessation Specialist, Registered Dietitian, CDE, Exercise Physiology certification, Lactation Counselor, NDPP Lifestyle Coach, CHWC.

Responsibilities

• Oversee a panel of members, assess health management needs and identify appropriate clinical interventions.
• Develop personalized care plans with long- and short-term goals and regular progress reassessments.
• Conduct outreach via phone, digital, or virtual means to improve health outcomes.
• Document activities in compliance with regulations and standards; participate in program improvements.
• Collaborate with multidisciplinary teams to support wellness coaching services and pilots.
