Job summary

Highmark Inc. seeks a wellness-focused professional to own a panel of members, assess health needs, and tailor interventions with a multidisciplinary team. You will develop personalized care plans, monitor progress, and coordinate care to improve outcomes across diverse populations.

You will also conduct outreach via telephonic or digital means, document activities per policy, and contribute to program pilots and new initiatives within a supportive, high-performing environment.

Qualifications

• Master's degree or equivalent experience in a relevant field.
• 3 years as a RD/Exercise Physiologist/Wellness Coach in a wellness-focused setting.

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards for coaching in tobacco cessation, weight management, sleep, stress, nutrition, activity and maternal health.
• Oversee a panel of members, assess health needs, identify clinical interventions and triage to resources.
• Create care plans with goals, remove barriers, identify internal/external resources, and schedule regular progress contacts.
• Demonstrate clinical outcomes and cost savings from wellness interventions across diverse populations.
• Ensure activities comply with business processes, regulatory and accreditation standards.
• Participate in workgroups to improve efficiency and member-centric wellness programs.
• Perform other duties as assigned.
