Highmark Health is seeking a skilled wellness coach to manage a panel of members with varied health needs. You will assess health management needs, create care plans, and coordinate referrals within a multidisciplinary team.

Telephonic and virtual outreach will support member engagement and outcomes. The role emphasizes evidence-based interventions across tobacco cessation, weight management, sleep, nutrition, and activity, with HIPAA-compliant documentation and quality improvement

Qualificações

• Master's degree or equivalent experience as required.
• 3 years as a Registered Dietician, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment.

Responsabilidades

• Develop personalized care plans and coordinate with multi-disciplinary teams.
• Outreach to members via telephonic, digital, or virtual channels to improve health outcomes.
• Document activities in accordance with regulatory and program requirements.
• Monitor quality outcomes (clinical, financial, and functional) for assigned member panels.
