Job summary
Highmark Inc. is seeking an experienced wellness/lifestyle coach to manage a panel of members with diverse health needs. You will develop personalized care plans, coordinate with a multidisciplinary team, and drive behavior change through telephonic, digital, or virtual outreach.
You will monitor outcomes, address barriers, and coordinate resources to support member health goals while ensuring compliance with regulatory and program standards.
Qualifications
- Master's degree required or relevant experience in lieu of master's degree as determined by the company
- Minimum 3 years as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused program
- Preferred certifications in tobacco cessation, diabetes education, CHWC or related wellness credentials
Responsibilities
- Provide high-quality lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health
- Oversee a panel of members, assess health management needs and triage to appropriate resources
- Create care plans with goals and regular contact to reassess progress
- Demonstrate clinical outcomes and cost savings from wellness interventions
- Ensure documentation complies with required standards and participates in program improvements
