Job summary

Highmark Health is seeking a dedicated wellness coach to oversee a panel of members, assess health management needs, and coordinate personalized interventions. You will use data and clinical judgment to guide lifestyle changes, connect members with multi-disciplinary resources, and monitor outcomes across diverse populations.

You will work with a multidisciplinary team and engage members telephonically or virtually to promote healthier behaviors and improve clinical, financial, and functional

Qualifications

• Master's Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or Clinical related field or relevant experience as determined by the company
• Minimum 3 years as a Registered Dietitian, Exercise Physiologist or Wellness/Lifestyle Coach in wellness/program delivery

Responsibilities

• Deliver evidence-based lifestyle coaching across tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health
• Maintain oversight of a panel of members; assess health management needs and refer to appropriate resources
• Create care plans with long- and short-term goals and regular contacts for progress reassessment
• Demonstrate outcomes and cost savings from wellness interventions
• Document all activities in compliance with policy, regulatory, and accreditation standards
• Participate in workgroups to improve efficiency, effectiveness and member-centric wellness services
• Perform other duties as assigned
