Job summary

Highmark Health seeks a Wellness/Lifestyle Coach to oversee a panel of members with varied health needs, providing personalized care plans and coordinating with multidisciplinary teams to improve health outcomes.

The role involves telephonic and digital engagement, development of individualized care plans, addressing barriers to care, and ongoing reassessment of progress in alignment with evidence-based wellness interventions.

Qualifications

• 3 years as a Registered Dietitian OR Exercise Physiologist OR Wellness/Lifestyle Coach in a wellness-focused program delivery environment

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards to deliver high quality lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Maintain oversight over specified panel by assessing health management needs and triaging to resources.
• Create care plans addressing identified needs with long- and short-term goals and regular follow-ups.
• Demonstrate clinical outcomes and cost savings from wellness interventions across diverse member populations.
• Ensure activities are documented in compliance with regulatory standards.
• Participate in workgroups to improve wellness coaching efficiency and program development.
• Other duties as assigned.
