Highmark Health is seeking a Wellness Coach to oversee a panel of members, assess health management needs, and design personalized care plans. You’ll work with a multidisciplinary team to coordinate resources and use data analytics to drive better health outcomes.

You will engage members telephonically or virtually, monitor progress, and document activities to meet quality and regulatory standards. Prior experience in wellness programs is preferred.

Qualifications

• Master's Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or Clinical related field OR relevant experience and/or education as determined by the company in lieu of master’s degree
• 3 years as a Registered Dietician OR Exercise Physiologist OR Wellness/Lifestyle Coach in a wellness-focused environment

Responsabilités

• Deliver evidence-based lifestyle coaching across tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health
• Develop personalized care plans and coordinate with multidisciplinary resources to support member health goals
• Engage with members via phone, digital, or virtual channels to improve outcomes and monitor progress
• Document activities in compliance with regulatory and accreditation standards
