Qualifications

• 3 years as a Registered Dietitian or Exercise Physiologist or Wellness/Lifestyle Coach in a wellness-focused environment.
• Experience in disease management or wellness program delivery preferred.
• Ability to coordinate with a multi-disciplinary team to support member health goals.

Responsibilities

• Maintain knowledge of evidence-based lifestyle interventions (nutrition, weight management, tobacco cessation, sleep hygiene, stress management, physical activity, maternal health).
• Assess health management needs for assigned member panel and identify appropriate clinical interventions or resources.
• Develop care plans with long- and short-term goals and regular contact for progress review.
• Demonstrate clinical outcomes and cost savings from wellness interventions.
• Ensure activities are documented per regulatory and accreditation standards.
• Participate in workgroups to improve efficiency and member-centric wellness services.
• Perform other duties as assigned.
