Responsibilities include clinical leadership during rounds, triage, admissions, documentation in EMR (PointClickCare), and regulatory compliance. The role requires RN licensure in CT and 2-3 years in SNF/LTC, with strong prioritization and communication skills.

Qualifications

• RN license in Connecticut (active, unrestricted)
• 2–3 years clinical nursing experience in SNF/LTC/sub-acute rehab
• Proven supervisory or charge nurse experience preferred
• Strong prioritization and independent decision-making during weekends

Responsabilités

• Weekend clinical leadership supervising nursing operations
• Conduct routine rounds across units monitoring resident status and workflows
• Provide rapid triage and emergency response for changes in resident conditions
• Assist with weekend admissions, transfers, and discharge orders
• Coach and support RNs/LPNs/CNAs to maintain shift accountability
• Ensure EMR documentation is accurate and compliant with regulations
