CVS Health is seeking a Minute Clinic Virtual Care Nurse Practitioner (Provider) to deliver holistic, evidence-based care through a remote platform for patients 18 months and older.

This full-time weekend-evening role requires an active Family Nurse Practitioner (FNP) license in the applicable states and at least two years of medically-relevant experience. Comprehensive benefits are provided, along with performance incentives.

Qualifications

• Must hold Master's Degree in Family Nurse Practitioner with current NBC and license.
• Minimum of two years of medically-relevant experience.
• Effective verbal, written and electronic communication skills.
• Strong organizational skills and ability to multi-task.
• Initiative, problem solving ability, adaptability and flexibility.
• Ability to work autonomously without direct supervision.
• Proficiency with information management and technology.
• Ability to collaborate with colleagues to provide quality care.
• Spanish language skills may be required in some markets.

Responsibilities

• Provide holistic, evidenced-based care including assessment, diagnosis, treatment and disposition planning for patients aged 18 months and older.
• Evaluate primary care, acute and chronic care patients and provide health education and counseling.
• Counsel patients on pregnancy prevention, STI prevention and contraceptive care.
• Educate patients on health maintenance and respond to inquiries.
• Document all patient care in an EHR per CVS Health policies.
• Coordinate care with internal and external colleagues to ensure high quality care.
• Work within a patient care team and advocate for patients and practice.
