Job summary

CVS Health is seeking a full-time Minute Clinic Virtual Care Nurse Practitioner (Provider) to deliver holistic, evidence-based care via a remote platform. You will collaborate with a dedicated team, independently providing assessment, diagnosis, treatment, and disposition planning for patients 18 months and older.

The role requires an active FNP license in eligible states, with at least two years of experience, strong communication, and the ability to work autonomously in a virtual setting.

Qualifications

• Licensed as a Family Nurse Practitioner in an eligible state (CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, WV)
• Minimum of two years of medically-relevant experience
• Excellent communication skills (verbal, written, electronic)
• Strong organizational and multi-tasking abilities
• Ability to work autonomously and with technology tools such as EHRs

Responsibilities

• Provide holistic, evidenced-based care including assessment, diagnosis, treatment, and disposition planning for patients 18 months and older
• Evaluate primary care and acute/chronic patients and provide health education
• Counsel on contraception and related care, and manage medications
• Educate patients on health maintenance and answer inquiries
• Document care in the EHR per clinic policies
• Coordinate care with internal and external colleagues within the care team
• Collaborate with Providers, physicians, pharmacists, and other care team members
• Work independently, prioritize, and advocate for patients
