CVS Health's Minute Clinic Virtual Care Nurse Practitioner role offers remote, full-time care delivery for patients aged 18 months and up. The position requires an active Family Nurse Practitioner license and the ability to obtain any additional licensure as needed.

The Nurse Practitioner will provide holistic, evidence-based care via a remote platform, collaborating with a multidisciplinary team to deliver timely, efficient care while documenting in the EHR and maintaining patient privacy.

Responsibilities

• Provide holistic, evidence-based care, including assessment, diagnosis, treatment, and disposition planning.
• Evaluate diverse patient populations aged 18 months and older in a primary care setting.
• Deliver contraception counseling and medication management.
• Educate patients on health maintenance and respond to inquiries.
• Document all care in the EHR per MinuteClinic policies.
• Coordinate care with internal and external colleagues to ensure high-quality outcomes.
• Collaborate with Providers, physicians, paraprofessionals and pharmacists.
• Work independently, prioritize issues, and advocate for patients.
