Job summary

CVS Health’s Minute Clinic Virtual Care Nurse Practitioner role delivers patient care through a remote technology platform. You will work with a dedicated team to independently provide holistic, evidenced-based care including assessment, diagnosis, treatment, and disposition planning for patients aged 18 months and older.

The position requires an NP license in listed states, at least two years of relevant experience, and the ability to work an evening shift with every other weekend.

Qualifications

• Mast. degree in Family Nurse Practitioner with current national certification.
• State licensure to practice in designated states (CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, WV).
• Two years of medically-relevant experience or equivalent.
• Strong communication, organization and adaptability.

Responsibilities

• Provide holistic, evidence-based care including assessment, diagnosis, treatment and counseling for patients aged 18 months and older.
• Evaluate primary, acute and chronically ill patients and coordinate care.
• Document all patient care in the EHR per MinuteClinic policies.
• Collaborate with Providers, physicians, pharmacists and other health care team members.
