Job summary

Highmark Health seeks an experienced wellness coach to oversee a panel of members with diverse health needs, designing personalized care plans and coordinating with a multidisciplinary team. You will use data and clinical judgment to identify interventions and engage members remotely or in person to improve outcomes.

Qualifications

• Master's degree or equivalent in a health-related field.
• 3 years as a dietitian, exercise physiologist, or wellness coach in a wellness-focused setting.
• Licenses or certifications may be preferred (see below).

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards for lifestyle coaching (tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity, maternal health).
• Oversee a specified panel of members, assess health management needs, and triage to resources.
• Create care plans with goals and regular contact for progress reassessment.
• Demonstrate clinical outcomes and cost savings from wellness interventions.
• Ensure documentation complies with regulatory standards.
• Participate in workgroups to improve efficiency and member-centric services.
• Perform other duties as assigned.
