Job summary

Highmark Health is seeking a skilled Wellness/Lifestyle Coach to manage a panel of members with diverse health needs. You will develop personalized care plans, guide behavior changes, and coordinate with a multidisciplinary team to drive outcomes.

This role requires a Master’s in a relevant field and at least 3 years of experience in wellness coaching or related health programs. Remote tools and HIPAA-compliant processes are used to support members.

Qualifications

Master's degree or higher in a relevant field or equivalent experience as determined by the company.
Minimum 3 years as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused setting.
Familiarity with evidence-based interventions and health coaching practices.

Responsibilities

Develop personalized care plans addressing identified health needs and goals.
Conduct outreach to members via telephonic, digital, or virtual means to improve outcomes.
Coordinate with multidisciplinary teams and refer to appropriate resources as needed.
Document all activities in compliance with HIPAA and internal policies.
Aim for quality outcomes across clinical, financial, and functional metrics.
