Insight Housing seeks a dedicated Case Manager to support homeless Veteran families by coordinating intake, assessments, and service plans. You will work with Veterans to secure housing, coordinate with landlords, and connect them with VA benefits and community services.

The role requires strong communication, the ability to work independently, and a commitment to helping clients achieve stable housing outcomes in a fast-paced environment.

Qualificações

• CPR/First Aid certification or training.
• Proficient in Microsoft Office (Word, Excel, Outlook, Teams).
• Experience or ability to work with HMIS is a plus.
• Strong communication and interpersonal skills.
• Ability to work independently and in teams during high-pressure situations.

Responsabilidades

• Coordinate intake and eligibility for new clients and maintain files.
• Provide outreach to homeless Veterans.
• Manage a case load and provide housing stabilization services.
• Coordinate with landlords and develop budgets and service plans.
• Link participants to VA benefits and other community resources.
• Maintain HMIS records and comply with confidentiality rules.
