Job summary

Center for Veterans Issues, Inc. is seeking a Case Manager to support Veterans experiencing homelessness in Wisconsin. You will provide strengths-based case management, link Veterans to housing resources, benefits, and community services to promote housing stability.

Qualifications

Bachelor's degree in social services (required) or an Associate's with 4–5 years of relevant case management experience.
At least 2 years of case management, assessments, counseling, and life skills support.
Experience working with Veterans strongly preferred.
Valid Wisconsin Driver's License, good driving record, and current auto insurance.

Responsibilities

Provide comprehensive, strengths-based case management to Veterans experiencing homelessness or housing instability.
Review intake assessments and identify individualized service needs.
Develop and monitor Housing Identification, Stability, and Single Coordinated Care Plans.
Partner with participants to set goals, execute participation agreements, and plan for long-term success.
Create thoughtful discharge plans that connect Veterans to ongoing community support.
Deliver housing stability counseling and connect Veterans to rental assistance, subsidies, and housing programs.
