Job summary

HELP USA is seeking a Case Manager to support homeless veterans in HELP USA transitional housing under the VA Grant per Diem program in Las Vegas. The role includes intake, assessments, and developing an independent living plan, plus advocacy for VA benefits, Social Security, and other public supports.

• Initial intake interviews and assessments to determine needs.
• Advocacy for VA benefits, Public Assistance, Social Security, and other public benefits.
• Identify barriers to housing stability and apply solution-based actions.
• Develop housing stability plans with steps toward permanent housing.
• Liaise with agencies and maintain a network of resources.

Responsibilities

• Conducting initial intake interviews and assessments and developing an independent living plan.
• Providing advocacy for public benefits including VA benefits, Public Assistance, Social Security, etc.
• Identifying barriers and challenges that veterans face and utilizing a solution-based approach to effectively resolve issues that threaten housing stability.
• Preparing housing stability plans with a path to permanent housing and self-sufficiency.
• Serving as liaison with appropriate agencies and programs and maintaining a network of resources.
