Job summary

Veterans Community Project seeks a Case Manager to engage and support Veterans experiencing or at risk of homelessness. You will provide flexible, Veteran-centered support in community and residential settings, helping with housing plans, accessing resources, and achieving long-term goals.

You will build trust, offer consistent guidance, and coordinate services to improve quality of life, navigate housing and recovery paths, and foster connections with local resources and partners.

Responsibilities

• Establishes rapport and builds trust with Veterans in shelters, encampments, temporary housing, and outreach programs.
• Assists Veterans in identifying immediate and long-term goals related to housing, income, health, and stability through collaborative assessment.
• Provides direct support and referrals for emergency needs such as shelter, food, hygiene supplies, and transportation.
• Assists Veterans in accessing health care, mental health treatment, substance use recovery services, and benefits enrollment.
• Gathers documentation for service eligibility, employment, or housing applications.
• Provides crisis intervention and de-escalation when necessary.
• Develops individualized case plans with goals in areas such as health, income, financial literacy, education, and community support.
• Coordinates with housing providers, landlords, and agency partners to secure housing opportunities.
• Delivers ongoing case management and coaching to help Veterans progress toward goals and sustain stability.
• Keeps timely records in files and logs, and tracks progress toward goals.
