Job summary

Catholic Community Services is seeking a Case Manager for the Veterans Services team. The role supports Rapid Rehousing for Veterans and their families, using Housing First and Progressive Engagement approaches to secure permanent housing.

The position involves outreach, client intakes, eligibility screening, and developing Housing Stability Plans. Requires one year of experience in social services and strong communication and organizational skills.

Qualifications

• Post-secondary education in a social services field with relevant experience preferred.
• Excellent interpersonal and written communication skills.
• Ability to work independently and in a team, under pressure.

Responsibilities

• Receive referrals from Coordinated Entry.
• Screen clients for Rapid Resolution and assess eligibility.
• Develop and initiate Housing Stability Plans.
• Assist with locating and securing housing.
• Input data into HMIS and maintain client records.
• Promote CCS mission and collaborate with community partners.
