Job summary

Insight Housing seeks a housing case manager for its VA Grants per Diem program, serving 18 Veterans. You will provide intensive case management, connect Veterans to medical and mental health resources, and assist with income and housing acquisition.

Under supervision, you will meet weekly with participants, maintain records, and support program compliance and reporting while promoting safety and recovery through a harm-reduction approach.

Qualifications

• High school required; Bachelor's degree in Social Services or related field a plus.
• CPR/First Aid training required.
• 1 year experience providing direct support services to homeless individuals or related social issues.
• Commitment to serving individuals experiencing homelessness and trauma-informed care.
• Proficient with computer and Microsoft Office; HMIS a plus.

Responsibilities

• Provide housing case management to Veterans in the program.
• Link Veterans to VA medical and mental health resources.
• Assist with income and housing acquisition.
• Meet with each participant weekly to monitor progress and provide support.
• Maintain case management files and assist with contract compliance and reporting.
• Promote safety and crisis intervention as needed.
• Drive to various sites as required and maintain program records.
