Job summary

Insight Housing in the Bay Area seeks a Case Manager to deliver housing stabilization services for homeless Veteran families under the Pathway to Housing program. Work includes outreach, intake, assessments, and coordinating VA benefits and housing placements.

Under supervision of the Program Manager, you will maintain case files, collaborate with landlords, and help participants achieve housing stability and independence through supported services.

Qualifications

• High school required. Bachelor's degree in Social Services or related field a plus.
• Ability to complete CPR/First Aid Training.
• 1 year experience providing direct support services to individuals experiencing homelessness, mental health, substance abuse or other social issues.
• Proficient in use of computer and Microsoft Office suite (Word, Excel, Outlook and TEAMS). HMIS experience a plus.
• Commitment to serving individuals experiencing homelessness.

Responsibilities

• Coordinate SSVF client intake process; conduct initial assessment and eligibility for new referrals.
• Provide street and venue-based outreach to homeless Veterans.
• Maintain a client caseload as assigned.
• Provide intensive housing and vocational case management to Veterans and their families.
• Conduct home visits as assigned.
• Develop budgets and service plans with each Veteran.
• Maintain HMIS database records and ensure data entry within 24 hours.
