Job summary

Veterans In Transition seeks a Case Manager to support the Veterans Services program. You will manage a caseload of households in Homeless Prevention/Rapid Re-Housing, conduct intakes, assess housing needs, and facilitate timely financial assistance or housing placement.

The role requires collaboration with VA staff, data entry into HMIS, and development of client-specific Transition Plans. Strong communication, organization, and teamwork are essential to stabilize permanent housing for veterans.

Qualifications

Bachelor’s Degree in a human or social services field, or 4 years of relevant experience.
Experience providing services to low-income/no-income households.
Ability to work in a team and promote positive team spirit.
Excellent interpersonal, oral, and written communication skills.
Strong organizational and time management abilities.
Proficient in Microsoft Word and database experience.
Ability to work with a diverse range of high-need individuals.

Responsibilities

Complete thorough intakes and eligibility screening with referred clients.
Initiate housing stability plan and coordinate landlord documentation as needed.
Enter initial information into HMIS and document ongoing service transactions.
Participate in the Wraparound Team to develop and monitor Transition Plans.
Maintain records, complete 90-day recertifications, and prepare case summaries.
Attend required meetings and trainings and share information with staff.
Other duties as assigned.
