Job summary

Jericho Project in Manhattan seeks a Case Manager to provide rapid re-housing and homelessness prevention services to low-income veteran families. This non-exempt, full-time role reports to the Assistant Program Director.

Based in NYC, you will conduct assessments, coordinate field and office visits, develop housing stability plans, and connect families with community resources and benefits. Compensation ranges between $53,000 and $55,000 annually.

Qualifications

• Bachelor’s degree in Human Services or related field.
• Experience in homelessness, substance abuse, mental health, or trauma settings preferred.
• Knowledge of NYC resources preferred.
• Strong organizational, writing, communication, and computer skills.

Responsibilities

• Provide rapid re-housing and homelessness prevention services to veteran families.
• Conduct assessments and intake for SSVF services; ensure documentation.
• Coordinate field and office case management; visit homes.
• Develop housing stability plans and link to community resources.
• Refer to public benefits and financial counseling.
• Maintain electronic case management system and file accuracy.
• Participate in outreach, meetings, and trainings.
• Collaborate with employment specialists for education and employment support.
• Mediate disputes and coordinate housing search and placement.
