Job summary

Jericho Project in New York City is seeking a Case Manager to deliver rapid re-housing services to veterans, young adults, and single adults. You will manage a caseload of approximately 18-20 participants and maintain frequent contact through in-person meetings, calls, emails, and surveys.

In this non-exempt role, you will work under the Assistant Program Director, coordinating housing searches, benefits referrals, and post-placement support to promote housing stability and independence.

Qualifications

• Bachelor’s degree in social services, human services or a closely related field.
• 0-6 months experience working in homelessness, substance abuse, mental health and/or trauma settings (preferred).
• 0-6 months experience working with veterans/military, young adults, or LGBTQ populations (preferred).
• Must possess strong organizational, writing, communication, and computer skills.
• This position includes fast paced case management and some field work.

Responsibilities

• Utilize Motivational Interviewing and Harm Reduction to assist clients in housing stability.
• Conduct assessments and program intakes, ensure documentation accuracy.
• Provide direct case management in office and during field/home visits.
• Develop and update housing stability plans and assist with housing search.
• Advocate with landlords and work with URRH housing specialist.
• Coordinate with Workforce Opportunities to link to employment resources.
• Refer to community resources for SSI/SSDI, healthcare, and other supports.
• Provide post-placement support and crisis intervention as needed.
• Maintain client files in the electronic case management system.
• Participate in community initiatives and team meetings.
• Carry out other duties as assigned.
