Job summary

Jericho Project in Manhattan is seeking a Case Manager to provide rapid re-housing services for veterans, young adults, and singles. You will manage a caseload of about 18–20 participants, conduct intake, and maintain timely documentation in our electronic system.

The role requires a Bachelor’s in Human Services or related field, strong organizational and communication skills, and experience with populations facing housing instability.

Qualifications

• Bachelor’s degree in Human Services or related field.
• Experience in homelessness, substance abuse, mental health or trauma settings preferred.
• Experience with veterans, young adults, or LGBTQ populations preferred.
• Strong organizational, writing, communication, and computer skills.
• Self-starter with ability to work with high-need participants.
• Knowledge of NYC resources preferred.

Responsibilities

• Provide rapid re-housing services to veterans, young adults, and single adults.
• Carry a caseload of approximately 18–20 participants.
• Conduct assessments and intakes; ensure documentation is complete.
• Update case notes in an electronic case management system promptly.
• Provide direct case management in office and during field/home visits.
• Develop and update housing stability plans to obtain or maintain housing.
• Assist with housing search, viewings, and neighborhood research.
• Coordinate with Workforce Opportunities to link to employment and education.
• Refer to community resources for benefits and services; SOAR training preferred.
