Job summary

The Jericho Project in New York, NY seeks a Senior Case Manager to provide rapid rehousing services to high-risk veterans, young adults, and families. You will manage a caseload of 10–12 participants and coordinate services with clinicians and staff.

In this non-exempt, full-time role, you will conduct intakes, develop housing stability plans, accompany clients to housing viewings, and advocate with landlords to secure and maintain housing. Location: Manhattan.

Qualifications

• Bachelor’s Degree in social work or related field required; Master’s preferred.
• Experience in homelessness, substance abuse, mental health, or trauma settings preferred.
• Experience with veterans, young adults, or LGBTQ populations preferred; NYC resources knowledge helpful.
• Strong organizational, writing, and communication skills; proficient with computers.

Responsibilities

• Use Motivational Interviewing and Harm Reduction to aid housing stability for veterans and families.
• Conduct assessments and intakes; ensure complete documentation and timely case notes.
• Refer participants to mental health services and coordinate housing applications with staff.
• Develop and facilitate workshops for independent skills and mental health.
• Provide direct case management in office and during field visits with high-risk participants.
• Develop housing stability plans and assist with income/benefits and budgeting.
• Coordinate with Workforce Opportunities to link participants to employment/education.
• Advocate with landlords/brokers to secure and maintain housing; liaise with housing specialists.
