Jericho Project in New York City seeks a Case Manager to deliver rapid re-housing services for veterans, young adults, and singles. The role manages a caseload of about 18–20 participants, with frequent contact through meetings, calls, and surveys.

The position reports to the Assistant Program Director and is based in Manhattan. The role requires a Bachelor's in Human Services or related field, and experience in homelessness, mental health, or trauma settings is preferred.

Qualificações

• Bachelor’s Degree in Human Services or a closely related field.
• Experience in homelessness, substance abuse, mental health and/or trauma settings preferred.
• Experience with veterans/military, young adults, or LGBTQ populations preferred.
• Knowledge of NYC resources preferred.
• Strong engagement and interpersonal skills.

Responsabilidades

• Provide rapid re-housing services to veterans, young adults, and single adults.
• Carry a caseload of approximately 18–20 participants.
• Maintain contact with participants via meetings, calls, email, and surveys.
• Develop housing stability plans and assist with housing search and placement.
• Coordinate with Workforce Opportunities to link participants to employment/education resources.
• Maintain accurate client files and complete required documentation.
