Job summary

American Family Care (AFC) is seeking an Advanced Practice Provider to care for patients within the scope of training and approved agreement. You will collaborate with other providers and staff to ensure efficient patient flow and high patient satisfaction.

Responsibilities include detailed health histories, diagnostic workups, therapy initiation, and referrals. Emergency management and patient education are essential components of this role.

Qualifications

• Excellent communicator with staff, patients, and family.
• Professional appearance and attitude.
• Board certification and licensure as PA or NP; current state license, DEA, and controlled substances certificate.

Responsibilities

• Perform complete, detailed health histories, review patient records, develop medical assessments, and order labs/imaging as appropriate.
• Formulate medical diagnoses and institute therapy or referrals to appropriate facilities or resources.
• Institute emergency measures and stabilization in emergencies (cardiac arrest, shock, hemorrhage, convulsions, poisoning, allergic reactions).
• Interpret and analyze patient data to determine status, management and treatment.
• Provide instructions regarding health care and health care promotion to patients and family.
