Job summary

American Family Care is seeking an Advanced Practice Provider (APP) to care for patients within the scope of training and approved agreement, collaborating with other providers at a franchised AFC location.

The role involves detailed health histories, medical assessments, and coordinating care across labs, imaging, and referrals, with emphasis on patient satisfaction and efficient flow in a franchise environment.

Qualifications

• ANCC, AANP, or NCCPA Board Certified – (with 1 yr ER/UC experience preferred)
• Active and current Physician Assistant or Nurse Practitioner licensure in the state of practice, DEA number, and state-controlled substance certificate, as required
• Able to multi-task and work independently
• Excellent communicator with staff, patients, and family
• Professional appearance and attitude

Responsibilities

• Perform comprehensive health histories and develop medical assessments
• Order laboratory, radiological, and diagnostic studies as appropriate
• Institute emergency measures and stabilize in critical situations
• Interpret patient data to guide management and treatment
• Provide health care guidance to patients and families
• Other duties as assigned
