Job summary

American Family Care, a leading urgent and primary care provider in the U.S., seeks an Advanced Practice Provider (APP) to care for patients within the scope of training and approved agreement under the governing board. The APP works with other providers and staff to maintain efficient patient flow and satisfaction at a franchised AFC location.

Responsibilities include comprehensive histories, diagnostics, diagnoses, therapy, emergency stabilization, and patient education.

Qualifications

• ANCC, AANP, or NCCPA Board Certified – (with 1 yr ER/UC experience preferred).
• Active and current Physician Assistant or Nurse Practitioner licensure in New Hampshire and state-controlled substance certificate.
• DOT Certification preferred.

Responsibilities

• Perform complete, detailed health histories and develop medical assessments.
• Order laboratory, radiological and diagnostic studies as appropriate.
• Formulate diagnoses and institute therapy or referrals to appropriate facilities.
• Institute emergency measures and stabilization when needed.
• Provide guidance on health care promotion to patients and family.
