American Family Care (AFC) is seeking an Advanced Practice Provider (APP) to care for patients within the scope of training and agreement, collaborating with other providers to ensure efficient patient flow and high satisfaction.

The position involves performing detailed histories, forming diagnoses, ordering tests, and providing patient education in a walk-in-based primary/urgent care setting. Certification and licensure in the state are required, with on-site responsibilities at AFC locations.

Qualifications

• Excellent communicator with staff, patients, and family.
• Professional appearance and attitude.
• Active PA/NP license in the state of practice with DEA and state-controlled substance certificate.
• Board certification by ANCC, AANP, or NCCPA (ER/UC experience preferred).
• Ability to multi-task and work independently.

Responsibilities

• Take comprehensive health histories and review patient records.
• Develop medical assessments and order necessary lab/imaging studies.
• Institute emergency measures and stabilize patients as needed.
• Interpret data to determine status and management.
• Provide health care guidance to patients and families.
