Job summary

American Family Care (AFC) is seeking an Advanced Practice Provider to care for patients within their licensed scope, collaborating with other providers to ensure efficient flow and high patient satisfaction.

The APP will perform comprehensive histories, develop medical assessments, and order appropriate studies, while delivering emergency care when needed and guiding patients and families on health care and prevention.

Qualifications

• Excellent communicator with staff, patients, and family.
• Able to multi‑task and work independently.
• Active and current Physician Assistant or Nurse Practitioner licensure in state of practice, DEA number, and state-controlled substance certificate, as required.

Responsibilities

• Perform comprehensive health histories, review records, and order labs and imaging as appropriate.
• Formulate medical diagnoses and institute therapy or referrals to appropriate facilities or resources.
• Provide emergency measures and stabilization in critical situations (cardiac arrest, shock, etc.).
• Interpret patient data to determine status, management and treatment.
• Provide guidance on health care and health care promotion to patients and families.
