Job summary

American Family Care (AFC) is seeking an Advanced Practice Provider (APP) to deliver high-quality care in a walk-in setting. You will work collaboratively with a team to ensure efficient patient flow and satisfaction across AFCs seven days a week.

Responsibilities include comprehensive histories, diagnoses, therapy, and referrals, with emphasis on emergency stabilization and patient education. Must hold PA/NP licensure, DEA, and state-controlled substance certification.

Qualifications

• Excellent communication with patients, families and staff.
• Professional appearance and attitude.
• Board certification as PA/NP required; active license and DEA/state certifications.

Responsibilities

• Take complete health histories and order appropriate tests.
• Develop medical plans and refer to appropriate facilities.
• Perform emergency measures and stabilisation as needed.
• Interpret data to guide patient management and treatment.
• Educate patients and families on health care and prevention.
• Additional duties as assigned.
